Sorry! This profile demonstrates a modest lymphocytic pleocytosis with marginally elevated protein. This could be suggestive of an inflammatory or autoimmune disorder, or perhaps a very mild or partially-treated viral encephalitis. We were correctly worried about an acute infectious meningoencephalitis. The patient's MRI demonstrates FLAIR hyperintensities within the dorsal pons. Altogether, this is suggestive of a rhomboencephalitis-- with our concern for infection, specifically an infectious rhomboencephalitis. Can you think of a common organism that would do this, and what CSF profile it'd create?
