Sorry! Yes, this patient is indeed at risk of developing seizures. She's on propofol since she just got intubated, so that's a little reassuring (but certainly doesn't rule out the presence of refractory status.) However, there's another more direct etiology that is a little more pressing and needs to be addressed first. Think back-- this patient had a large MCA infarct occupying a majority of that vascular territory. We can't ignore the burgeoning structural abnormality of cerebral herniation. What can we do to address this underlying cause?
