The patient's BP doesn't respond to the initial 1L. You give the remaining 30mL/kg bolus, with good response. Vasopressors do not need to be started. Cultures are drawn. IV vancomycin and piperacillin/tazobactam are started.

One hour later, you're called back to see the patient. She is hypotensive to 85/40, with HR 160s-180s. Rhythm remains a-fib.
