Very close. It's true that this patient has a clear acute SDH causing cerebral herniation. The SDH on the right side is causing the most trouble, and given its sheer extent, burr holes would likely be inadequate for evacuation of this hematoma. A craniotomy is likely to be needed, and you can help weigh in on this. Fortunately, a hemicraniectomy is unlikely to be needed at this juncture. Regardless, there's another option that takes even higher priority than this definitive intervention.
